National project to develop and test models for large scale patient partnering approach to Value Based Diabetes Care and use of Patient Reported Outcomes

To identify practical ways by which the Danish healthcare system can deliver better health and quality of life for people with diabetes across the lifespan for the same or less resources by focusing more on what matters the most the person with diabetes and their family at each step.
To adopt and apply systematic and evidence-based methodologies for partnering with patients and caregivers across all phases of the project.

The Danish value-based health care initiative has several tracks with the common goal of increasing the value of care for patients by spending resources more wisely, and by redesigning the organization of health care and treatment.
The Danish regions want to develop a more value-based healthcare system where impact of treatment is measured as person-centred patient-outcomes.
Therefore, in a national project, the regions have decided to initiate attempts to steer and manage healthcare based on what creates value for the patient at the lowest costs.
Seven treatment areas have been selected for trials: among these diabetes, anxiety and depression, prostate-cancer and epilepsy. An essential part of the project will be to develop and test a BI-solution that can systemize and present data (both PROM and clinical) for hospital management and clinicians in a comprehensible and operational way.

The national diabetes project adopts a systematic partnering approach to patient partnering with the national patient association, local patient associations and indiividual patients.
The program adopts components from the model of Value Based Health Care (VBHC) defined by Michael Porter which are relevant to a European healthcare system. The project is nationally orientated, to involve all regions in Denmark and a working group has met the first time with representatives of patient organisations, municipalities, primary care practice, psychology and multi-disciplinary professions from secondary healthcare to ensure that the foundation for future work is agreed upon, ie. an operational person-centred outcome model that can support cross-stakeholder collaboration for better treatment outcomes and health innovation.

Nøgleresultater

The national working group for value based diabetes care agreed on a minimal set of key person-centred outcome domains for application i type 1 and type 2 diabetes in a cross-sectorial testing set-up. The importance of use of Patient Reported Outcomes was established in this work as an important complement to the improved use of clinical outcome data for ongoing quality improvement. The project focuses on establishing a broadly supported diabetes health outcomes model and decision-support and care transformation model with extensive use of PRO that can effectively support decision-making for improved value generation at all levels in the healthcare system. Value is defined as the overall health effect over the total costs of care for that person.
