Description of impact
In the project Future Patient, data about citizens with chronic heart failure have been collected using an online portal. With these data it is possible to monitor the citizens' health, and health personnel can then react more quicker. This project contributes with a social impact or the included patients, as they feel more secure in their condition when they know that healthcare personnel is monitoring their data and reacting to exacerbations. There is also a quality of life impact, as patients describe how they can more easily avoid getting admitted to the hospital by being aware of exacerbations of their conditions, thus being able to act on these.| Category of impact | Social impact, Quality of life impact |
