Når en ’fremmed’ dør - horisonter, mentalitetsrammer, samtidigt nærvær og belonging i sygdoms- og plejekonteksten
Baggrund
Afhandlingen undersøger betydningen af migration for udformningen af sygdoms- og plejekonteksten med livstruende sygdom og livsafslutning. Mere specifikt har afhandlingen til formål at skabe en sociologisk forståelse af hvordan palliative professionelle oplever at migrationshistorier (livserfaring med migration) har betydning for plejekontekstens udformning samt en sociologisk forståelse af hvordan migrationshistorier har betydning for individets håndtering og udformning af sygdomsforløbet og for individets håndtering og udformning af livets afslutning som følge af livstruende sygdom. Afhandlingen er et sociologisk bidrag til det tværdisciplinære forskningsfelt Humanistisk Palliation, som den er indskrevet i.
Teori
Afhandlingen har afsæt i en processuel forståelse af migration, i overensstemmelse med begrebet, ’migrancy’ (Ålund og Schierup 1987). Ifølge denne forståelse skal migration ikke blot betragtes som en enkeltstående bevægelse fra et land til et andet, på et givet tidspunkt, men som en proces der involverer tiden hen imod migrationen samt den efterfølgende tid med integration og tilpasning til migrationslandet og efterfølgende transnationale bevægelser (Ålund og Schierup 1987). Der er altså tale om en forståelse af migration som en erfaring, der kan have mere eller mindre indflydelse på individets livsverden og håndtering af hændelser selv lang tid efter selve flytningen. Afhandlingen har i overensstemmelse hermed afsæt i et dynamisk kulturbegreb. Kulturen udgør på den ene side individets tolkningsskema, men på den anden side absorberes og formes kultur af individet i overensstemmelse med de sociale positioner det indtager og det levede liv (Hastrup 2007 og Eriksen og Sørheim 2005).

Metode
Metodisk er der tale om en kvalitativ tilgang med afsæt i det hermeneutiske paradigme og den forstående forskningstype (Launsø og Reiper 2005). Der søges med afsæt i det hermeneutiske paradigme og Hans Georg Gadamers hermeneutiske filosofi, en forståelse for de forskellige horisonter i plejekonteksten samt hvilke erfaringer, der danner grundlaget for disse horisonter og hvordan de får betydning i plejekonteksten. Der er tale om en adaptiv tilgang, idet der veksles mellem afsæt i den eksisterende viden (forforståelse) og en ny forståelse af den eksisterende viden med tilføjelsen af et nyt teoretisk blik samt udvikling af den eksisterende viden gennem generering af ny empiri.
Der er til undersøgelsens formål foretaget semistrukturerede interviews med professionelle fra det specialiserede palliative felt samt med læger og sundhedsprofessionelle fra andre dele af sundhedssystemet, som også har erfaringer med pleje og behandling af etniske minoritetspatienter med livstruende sygdom og til livets afslutning. Der er foretaget semistrukturerede narrative interviews med kræftpatienter og tidligere kræftpatienter samt pårørende. De interviewede patienter og tidligere patienter har alle indvandrerbaggrund med oprindelse i både vestlige og ikke-vestlige lande.

Analyse
Analyseramme A behandler spørgsmålene om de professionelles erfaringer og analyseramme B behandler spørgsmålet om patienter og tidligere patienters oplevelser og håndtering af sygdom og livsafslutning. Til analyse af interviewmaterialet, er der anvendt en sammensætning af antropologisk og sociologisk kulturteori, hverdagslivssociologisk teori, interaktionistisk teori samt teori om belonging. Der er tale om orienterende begreber som sættes i spil med empirien. Ved hjælp af kulturteorien (Hastrup 2007; Eriksen og Sørheim 2005; Gunaratnam 1997) og teori om fremmedhed (Schutz 2005) belyses de professionelles kulturforståelser og deres oplevelser af forskellighed. Med afsæt i teorien om statusovergange (Glaser og Strauss 1971) belyses den sociale organisering omkring sygdomsforløbene. Med afsæt i teori om hverdagslivets forandrede betingelser (Bech-Jørgensen 1994) belyses de forskydninger, skred og brud som individet oplever som følge af sygdomsforløbet samt individets håndtering heraf. Med teori om belonging samt teori om kost som symbol på belonging belyses individets tilhør til forskellige fællesskaber med henblik på at identificere migrationshistoriens manifestering i den sociale organisering samt håndtering af ydre og indre aspekter af sygdomsforløbet og til livets afslutning. Analysen fremhæver samspillet mellem det almene og det partikulære på tværs af kultur.

Konklusion
De professionelle erfarer at migrationshistorier manifesterer sig som anderledes kulturelle praksisser, anderledes kommunikative praksisser samt traumatiske livsafsnit der får betydning for udformningen af plejen og udformningen af dødskonteksten (Analyseramme A). Patienternes migrations- og sygdomsnarrativer viste, at migrationshistorien manifesterer sig som ønsker om autentisk mad fra oprindelseslandet, særlige overvejelser over udformningen af det samtidige nærvær og social responsivitet under sygdomsforløbet og særlige overvejelser over begravelsessted og ritualer (Analyseramme B). I overensstemmelse med anden forskning, konkluderer afhandlingen også at palliativ pleje virker fremmende for kulturel sensitiv pleje, i nogle dele af plejeforløbet henimod livets afslutning, mens den kan have svære vilkår i andre dele, fordi der kan forekomme forskellighedsproblematikker i relation til de etiske principper for pleje og behandling mellem de professionelle og de pårørende/patienten. Kommunikation i plejekonteksten er, ikke overraskende, særlig vigtigt for ydelsen af kulturel sensitiv pleje. Det konkluderes slutteligt at et teoretisk afsæt i begrebet migrationshistorier tilføjer ekstra dimensioner til analysen af kulturel forskellighed i forbindelse med livstruende sygdom og livsafslutning. Nemlig den livshistoriske dimension. Migrationshistorien har betydning for både vestlige og ikke-vestlige etniske minoriteter, idet sygdomsforløbet og livets afslutning indebærer overvejelser over ’nært’ of ’fjernt’.

English Abstract
When a ’stranger’ dies - horizons, frames of mentality, simultaneous presence and belonging in the illness- and care context.

The PhD thesis explores the meaning of migration in the illness- and care context of life threatening and terminal illness. The Objectives of the thesis are to create a sociological understanding of how palliative care professionals experience the influence of migration histories on the care context and to create a sociological understanding of how migration histories influence the individuals’ coping with illness and shaping of the illness trajectory and towards end-of-life. The Thesis is a sociological contribution to the interdisciplinary research field of Humanistic Palliation in which context it has been written.

Theory
The underlying understanding of migration is based on a processual concept of migration, consistent with the concept ‘migrancy’ (Ålund and Schierup 1987). According to this concept migration is to be understood not as a singular event but as a process involving the time before migration and after migration and considering the process of adapting to the country of migration and subsequent transnational activity too. In this sense migration is to be understood as an experience which has more or less influence on the individual life world and coping with events even after the actual move. In accordance with this understanding of migration the thesis applies a dynamic understanding of culture. Culture is thus to be understood on one hand as an individual’s schemata of interpretation (a structure) and on the other hand culture is absorbed and shaped by the individual according to its social positions and lived experience. Culture is changeable over time in society and in the individual (Hastrup 2007; Eriksen and Sørheim 2005). Hence culture in this sense is to be understood as a flexible structure.

Methods
The thesis makes a qualitative approach with an offset in the hermeneutical paradigm and thus the type of research that seeks understanding (Launsø and Reiper 2005). With an offset in the philosophical hermeneutic paradigm and Hans Georg Gadamers understanding hereof the aim is to understand the different horizons present in the care context; what frames the horizons and how this is influencing the illness- and care contexts. The methodological approach is adaptive because of the dialogical relationship between prior understanding and new understanding and between theory and data. Qualitative data was collected by semi-structured interviewing with health and palliative care professionals and semi-structured narrative interviewing with cancer patients and former cancer patients. The interviewed patients and former patients have migrant background and originated from both western and non-western countries.

Analysis
One part of the analysis concerns the analyses of the professionals’ experiences and the other part concerns the analyses of the patients’ and former patients’ migration- and illness narratives. To inform the analysis anthropological and sociological theories about culture along interactionist theory, everyday life theory and theory about belonging have been used. With an offset in theory about status passage (Glaser and Strauss 1971) the social organization of the care context and the illness trajectory was explored. Theory of strangerness informed the analyses of the professionals’ position in the care context when patients and relatives showed a different cultural approach or when language barriers affected communication. A theory of the change of conditions of everyday life (Bech-Jørgensen 1994) was used to inform the analysis of illness narratives and how the individual copes with changes. Theory of belonging and theory of food as a symbol of belonging, was used to inform the analysis of patients identification with people and places and hence the manifestation of the migration history in the social organization of the illness trajectory, and in coping with the psychosocial and structural aspects of the illness trajectory. The analyses emphasize common as well as particular aspects of living with life threatening and terminal illness across cultural groups.

Conclusion
The PhD thesis concludes that professionals in palliative care contexts experience the manifestation of the migration history partly as culturally different practices and different ways of communicating about terminal illness and dying, and partly as traumatic life experiences which influences the care needs and the shaping of the dying context (Analyseramme A). The thesis also concludes that among patients with migrant background the migration history manifests as preferences for certain authentic food which they know from the country of origin or their upbringing in the country of origin. Also particular considerations concerning the organization of the simultaneous presence of significant others, and considerations of place of burial and rituals were forthcoming (analyseramme B). Consistently with existing research (chapter 3) the thesis concludes that palliative care is ideal for the provision of culturally sensitive care in relation to some parts of the illness trajectory more so than in others. Thus the analysis showed that in some parts of the illness trajectory, e.g. when the illness is terminal, the principles and ethics of palliative care may contradict the principles and ethics of care and treatment in some cultural groups. An emphasis on the importance of communication is, not surprisingly, of particular importance in the provision of culturally sensitive care. Finally the thesis concludes that the concept ‘migration history’ positively adds the life course dimension to the analyses of cultural difference which can help enlighten the perspective of cultural variation and the relationship between culture and life course and thus how these intertwine in the illness and care contexts. The analyses showed that the migration history adds meaning to the illness and dying trajectory to both western and non-western migrants as individuals from both groups expressed considerations or concerns about ‘here’ and 'there'.
